We like to maintain accurate, up-to-date information on all our patients and their dental conditions. When new patients first arrive for their appointment, our office will ask that you supply certain details such as:

• patient's name and address
• person responsible for payment
• medical history & current prescriptions
• your insurance plan (if applicable)
• consent for examination & treatment
